What Is It?

Use this form to request aggregate institutional data that is not available in an existing report or dashboard. Your request should have clearly defined populations, metrics, and time frames.

This form should not be used for:

• Ask general questions (submit a General Inquiry)
• Report issues about the website or data (submit Report an Issue)
• Exploratory or open-ended questions (submit an Analysis Request)
• Requests for training or consultation (submit a Consultation & Training Request)
• Requests for data about employees involving race or ethnicity (submit a UHR Data Request)

Important Notes

• Submission of a request does not guarantee fulfillment. Requests are reviewed based on institutional priorities, data availability, and resource capacity.
• Additional clarification may be required before work begins.
• All communication regarding your request will take place via TeamDynamix. You can respond to the automated emails you will receive from TeamDynamix by logging in to TeamDynamix and adding comments to your ticket.
